Directions to Registering a Death

1. Pick up the death Certificate from the family doctor or hospital.

2. Go to the Registrar of Deaths.

3. The Registra will want the Death Certificate from the Doctor, the deceased's medical card (if you have it at hand), and any War Pension Books.

 
The Registra will require to know:-

The date and place of death.

The deceased's last (usual) address.

The deceased's full name and maiden name (if a married woman).

The deceased's date and place of birth.

The deceased's and spouse's (if married) occupation.

Whether the deceased was in receipt of a pension, or an allowance from public funds.

If the deceased was married, the date of birth of the survivng spouse.

 
The Registra will give you:-

(a) The Green Certificate for the funeral Director.

(b) The Death Certificate for the D.S.S.

(c) Any additional copies of the Death Certificate, which will cost a few pounds.
